  STATE ENTRY CHECKLIST

  1. Attach State Entries as shown below.
  2. Swimmers: Hy-Tek Entry/Proof of Time Report:  Quals must be recorded with the VarVee State Qualifier Database.
  3. Divers: Hy-Tek Entry will serve as your entry form.  Copies of your qualifying dive cards need to be at the CHSAA office.
  4. Qualifying Times from a Relay Lead Off Split: Submit that meet result with your state entries and indicate you are using a lead off split.
  5. Submit Cover Sheet & Gatelist (Interactive form below)
  6. DUE:  Sunday, May 16th at 12:00 pm (noon)
